Internal Safety Reporting and Incident/accident Response

(a) Each program manager must establish an internal anonymous safety reporting procedure that fosters an environment of safety without any potential for retribution for filing the report.

(b) Each program manager must establish procedures to respond to an aviation incident/accident.

Section 91.1021 — Internal safety reporting and incident/accident response — requires the program manager to maintain an internal safety reporting system and an incident/accident response capability.

Safety management: Most major fractional programs operate Safety Management Systems (SMS) approaching the standard required for Part 121 carriers. Confidential reporting channels, hazard identification processes, and corrective action tracking are required.

Incident/accident response: The program manager must have an incident response plan covering emergency notification, NTSB/FAA reporting, family communications (for passenger incidents), media response, and operational continuity.
