This subpart prescribes flight rules governing the operation of aircraft within the United States and within 12 nautical miles from the coast of the United States.

Section 91.101 establishes the applicability of Subpart B — the flight rules. The reach is everything Subpart B says: aircraft within the United States, including its territorial waters, plus N-registered aircraft operated by U.S. citizens outside U.S. airspace (except where contrary to local law).
Subpart B is the operational backbone of Part 91. Every reg that governs how you fly — preflight action, right-of-way, airspace, altitudes, fuel, weather minimums, flight plans, instrument approaches — sits in Subpart B. If you can recite § 91.103 through § 91.193 from memory you can pass most of a checkride.
